Horizon EXTRA Benefits Card

Your Horizon EXTRA Benefits Card is pre-loaded on the first of every quarter (January, April, July, October) with a $400 allowance that you can use to purchase OTC and health-related items from participating retail stores. That’s up to $1,600 a year to buy personal health items from stores like Walmart, Walgreens, CVS, Dollar General, ACME, Rite Aid and more.

Two ways to shop:

  • Visit a participating retail store. Pay for your items at the cashier, not the pharmacy window.
  • Have your items shipped to you. Visit HorizonExtraBenefits.com or call 1-800-480-6598 (TTY 711) for home delivery at no cost to you.

Eligible items up to your available balance amount will be covered. For a full list of participating retailers and covered items, visit HorizonExtraBenefits.com or call 1-800-480-6598 (TTY 711).

Important: Any unused balance will not rollover. You must use your credit amount before the end of each three-month period.

Special food benefit for eligible members*

Members who qualify as chronically ill and participate in the Care Management program can use their Horizon EXTRA Benefits Card to purchase food and produce. Eligible items include:

  • Bottled water
  • Breakfast foods
  • Dry pasta
  • Fruits and vegetables
  • Produce
  • And more!

Members will have the ability to purchase fresh fruit via Farmbox and online through a retailer.

Reach out to your Care Manager or call Member Services to find out if you qualify. Once you become eligible, you will be able to use your OTC Benefit Card to purchase select healthy foods at the start of the next quarter.

Utility benefit*

Eligible members also get an extra $75 per quarter (every three months), $300 per year, on your Horizon EXTRA Benefits Card for utilities, if you qualify. Utilities are the basic services for your home or apartment, like water, electric and gas. Gasoline from a gas station is not included.

How to pay:

  • Phone: Call your utility provider and use the card as a form of payment.
  • Mail: Enter the card number as a form of payment on your billing statement and return it to your utility provider.
  • Online: Go to your utility provider’s website and use it to make a payment online.

If your benefit does not cover the full amount, contact your service provider to submit an alternate payment option. If you have issues or your service provider does not accept the card, please call Horizon EXTRA Benefits Card Member Services at 1-800-480-6598 (TTY 711) or visit HorizonExtraBenefits.com.

* The benefits mentioned are part of a special supplemental program for the chronically ill. Not all members qualify.

OTC Benefit Catalog

Every three (3) months you will receive an additional $270 credit (up to $1,080 yearly) to shop from the OTC Benefit Catalog. The Catalog has personal health items, including dental care, first aid kits, heat wraps, sleep aids, blood pressure monitors, vitamins and more! A full list of items is available in the OTC Benefit Catalog.

Be sure to use your benefit amounts before the end of every quarter (March, June, September and December). There are three ways you can order:

  • Over the phone: 1-800-480-6598 (TTY 711)
  • Mail: fill out and mail one of the order forms provided in the catalog
